Appointment Scheduling Tips
- If you are new to Boost Mode, please schedule a new patient consultation or weight management consultation. This will be with one of our nurse practitioners. This visit may be in-person or virtual.
- Visits are approximately 50 minutes and are good for one year. There is a consultation fee of $125.
- It is mandatory in the State of Ohio that all new patients receive a good faith exam (GFE) completed by a nurse practitioner or other medical provider. This exam makes sure you are medically appropriate and a good candidate to receive the treatment you choose. Boost Mode is unable to provide a service to you without this GFE being done prior to your first visit.
- If you are a current patient looking for information or to change the type of service you are receiving, please schedule your appointment for a re-evaluation service.
- If you are a current patient of Boost Mode, please schedule your desired service. Please review the staff availability as we have multiple staff who are available to provide services on different days.

Weight Management
There are some options to help with managing your weight. For details schedule a new consultation for weight management to review what option is the most appropriate for you.
Meet with our nurse practitioner so we can individualize our service(s) to meet your goals.
Once the appropriate program has been chosen, you will schedule your appointments with one of the nurses who are well versed in helping you to reach your weight goals.
The cost depends on the service chosen. This will be agreed upon prior to starting our weight management program.

Bloodwork
We have the capability of doing labs on you as deemed appropriate by our nurse practitioner. The order must be approved by our nurse practitioner prior to scheduling the appointment.
What to expect:
- Meet with the nurse practitioner to discuss your need
- Orders will be written for appropriate bloodwork
- An appointment is scheduled (there will be a lab draw fee)
- Once the results are available they will be reviewed by our nurse practitioner then sent to you through your medical record or reviewed with you by our nurse practitioner
- Cost will be reviewed with you prior to you ordering them
- You and your nurse practitioner will develop a plan moving forward to address the results of the bloodwork
