Boost Mode Spa memberships are for new patients or existing patients that are due for their annual exam. The membership includes the initial visit fee, lab review fee and membership set up. For existing patients, this fee will be waived since you are already up to date on mandatory monitoring.
A 90 day commitment is required prior to cancellation, and memberships will auto renew until canceled either by patient or staff. Unused services do not roll over to the next month. Memberships can be put on hold for 30 days every 6 months with at least a 48 hour notice to staff. For cancellations, a 30 day notice is required.
